ABOUT THE MEDICAL CARD:
• The medical card is a personal “Medical History” card provided to patients registered at the Sağlam Ailə Medical Centers network.
BENEFITS:
Cardholders receive:
• One free test per year: either a Complete Blood Count or a Semen Analysis,
• A “Your Age = Your Discount” offer during their birth month,
• 5% to 20% cashback in bonuses on the annual amount spent, added to their medical card.
TERMS OF USE:
• The card is personal and cannot be used by others.
• To activate bonuses, the cardholder must present valid ID.
• Accumulated bonuses can be used during check-up packages or other medical tests.
• Patients may pay for services partially with bonuses and the rest with an additional payment.
• If the bonus amount exceeds the service cost, the remaining bonus stays on the card and can be used later.
• Bonuses and free tests can only be used by presenting an ID.
• Bonuses can be used all at once or gradually, depending on the patient’s preference.
• If the card is lost, the patient can restore it for a fee by visiting any Sağlam Ailə branch with valid ID.
• The restored card will contain the full medical history and bonus balance previously linked to the patient.
• Sağlam Ailə Medical Centers reserve the right to make changes to the terms of use.
